customizing GW7 WebAccess signon page

Can anyone point me to instructions on removing some of the options on the GW7 WebAccess signon page? I'd  like to only have  the username and password boxes available and remove the rest of the boxes - language, connection speed, remember, etc.

It's been a long weekend and this is the final part to get done.

Any leads are greatly appreciated.
