We use Proofpoint (which is cloud based).
Pros:
High quality spam detection.
Every user gets their own personal block lists and pass lists.
If malware does get through (other Proofpoint customers find that an
email included an infection), we administrators get an email
notification that the thing was delivered to the end-user.
Cons:
Expensive.
Interface can be quite confusing to figure out what applies to the mail
flow processing, where.
When an email is automatically encrypted because it contains HIPAA (or
other PII), some outside recipients have trouble retrieving the email.
The first time they receive an email, they have to create an account
with password. Later, they have forgotten their password. The password
recovery process does NOT work, if the user is behind certain web proxy
firewalls. The only thing that can be done is to delete their account,
and they create a new one with a password they remember.
In answer to the last question: yes, we are happy with it.

Cloud based or on-prem? Any pros/cons ?
